Training - Forms


Prospective approval of Advanced Training (AVT)

Trainees must forward an application for approval of their proposed training post for assessment by the Training Committee within three months of commencing in the post, but preferably prior to their commencement date.

In-Training Assessment (ITA)

All trainees must have assessments completed by their Supervisor of Training.  For basic training, an annual assessment form should be completed, with 6 month assessments required for advanced training.

Final In-Training Assessment (FITA)

Once trainees have completed all their Core Intensive Care training, a FITA must be signed off. It is essential that this form is completed by the SOT who supervised the last Intensive Care training post.
