Training - Admission to Fellowship
Trainees who have completed all the required training components must ensure that a Final In-Training Assessment (FITA) form is completed and signed off by their last Intensive Care Supervisor of Training. All outstanding fees must be paid at this time.
In order to apply for Fellowship, Trainees are required to complete the Admission Authorisation Form.
Australian citizens and permanent residents of Australia need to complete the Medical Practitioner Information Form (HIC Form).
(These forms can be found below).
Upon receipt of all relevant documentation the admission process will begin. Applications take approximately 4-6 weeks to be processed.
